Data: Submission, Dictionary, and Exceptions

Please read the full detail for each data element as described in the data dictionary. Follow the Description, Codes and Values, as well as Notes for Abstraction for each data element/variable. Be sure to use the appropriate data dictionary version which aligns with the discharge date for which you are reporting. Each variable has a specific Data Element Name as well as accepted format. Some of the variables are mandatory to report, for example, date of birth (DOB); others are situational. For example, conditional responses include a datetime to align with a response of yes to antibiotic administering antibiotic. Sections:
  1. The Description section gives you the definition and detail information of each variable, i.e. , what the data element/variable represents.
  2. The Codes and Values section provides accepted values and the corresponding clinical/technical meaning for each value.
    • Note that only the listed values for variables will be accepted in the portal. Please read the clinical meaning for each code thoroughly and select the correct value of each variable for each case. The accuracy of these values is important, as they are used in measures/bundles calculation.
  3. The Notes for Abstraction section gives you important information for data abstraction. Clinical directives and correlations between related variables are provided. Different clinical scenarios for reporting variables are listed; constraints and conditions (sufficient and/or necessary) between correlated variables are stated.
